Falls Risk Assessment and Balance training
If you’ve experienced a fall, feel unsteady on your feet, are fearful of falling, or simply want to stay safe and mobile, this service is designed for you. We provide comprehensive in-home assessments to identify both personal and environmental risk factors, following world-leading guidelines for falls prevention. From there, we create a tailored plan to reduce or remove hazards - this may include a personalised 1:1 physiotherapy and balance program, a structured home exercise routine, and equipment recommendations, and practical strategies to support safer movement at home and in the community. We also work closely with your broader care team to help create a shared approach that supports your safety, independence, and confidence.
Neurological Assessments
We offer specialised in-home assessments for people living with neurological conditions such as stroke, traumatic brain injury (TBI), dementia, Parkinson’s disease, and motor neuron disease. During your initial appointment, we assess mobility, strength, coordination, and overall function to understand how your condition affects daily life. From there, we develop a personalised 1:1 physiotherapy program that works toward your individual goals - whether that’s improving mobility, maintaining strength, reducing stiffness, or staying independent with everyday tasks. We also provide practical education and hands-on strategies for family, carers, and support teams to assist confidently with movement, transfers, positioning, and physical care
Transfer and Manual Handling Assessment
We provide assessments to review how you currently transfer and mobilise - whether that’s getting in and out of bed, moving between surfaces, or walking with support. We identify any risks and recommend equipment that can improve safety, comfort, and independence. We also offer hands-on education and practical training for family members and support teams - delivered in person, in writing, or a combination of both - to help everyone assist safely and confidently. Our goal is to support safe movement for the person receiving care, while also protecting the wellbeing of those providing it. We create clear, tailored resources to guide ongoing support and ensure everyone stays informed and empowered.
Equipment Prescription
We begin with a thorough in-home assessment to understand your mobility, comfort, and safety needs. This includes reviewing how you move through daily tasks and identifying equipment that may support you - such as walking aids, transfer aids, seating solutions, sleep and positioning systems, and pressure care equipment. If needed, we can arrange follow-up appointments at your home or with equipment providers to trial recommended items. Once equipment is delivered, we offer practical education to ensure it’s used safely, confidently, and in a way that truly meets your needs.
Reablement
This service is ideal for those who want to increase their physical activity and regain strength to stay living at home, participate in meaningful activities, and feel more confident in the community. We begin with a thorough in-home assessment to understand your current abilities, goals, and any barriers to movement. From there, we create a tailored physiotherapy program - delivered 1:1 with your physiotherapist - to help rebuild strength, improve endurance, and support safe, active living.
Manual Handling Training
We offer manual handling training that can be tailored to an individual client or delivered as general education for teams supporting multiple clients. Sessions include practical guidance on safe movement techniques and hands-on practice with commonly used transfer aids - alongside clear, accessible theory to support injury prevention and confident care.
